A retrospective analysis of three trials studying beta-blockade to prevent cardiovascular events in at-risk patients undergoing non-cardiac surgery suggests that the regimen studied did not significantly increase stroke risk.

Peri-operative beta-blockade has been widely used in patients at risk of cardiovascular events who are undergoing non-cardiac surgery, as evidence suggests that it reduces the risk of serious cardiac events. A large controlled trial, however (the POISE trial; Lancet 2008; 371: 1839-47), found that the regimen used in that study, metoprolol m/r 200mg daily, was …

A 58-year-old man presented to the emergency department with sudden onset rapid palpitations and significant presyncope while walking on the flat. The previous day he had undergone DC cardioversion for atrial fibrillation (AF) which had been initially successful. However, 6 h after cardioversion he was aware of intermittently raised but regular heart rates. On arrival at the emergency department (ED) he was well with no haemodynamic compromise. The ECG showed an atrial tachycardia instead of AF. Medications consisted of propafenone 300 mg twice daily, Bisoprolol 5 mg at night and …

In patients with hypertension, β blockade decreases muscle sympathetic nerve activity (MSNA; micrographic technique) expressed in burst frequency (burst/min) but does not affect MSNA expressed in burst incidence (burst/100 heart beats), because reductions in blood pressure (BP) upon each diastole continue to deactivate the arterial baroreceptors, but at a slower heart rate (HR). We studied the effects of oral β blockade on MSNA and baroreflex sensitivity (BRS) in normal participants. Bisoprolol (5 mg, 1 week) was administered in 10 healthy young adults, using a double-blind, placebo-controlled, randomized cross-over study design. …

CRD Summary: This review concluded that the magnitude of heart rate reduction was statistically significantly associated with the survival benefit of beta-blockers in heart failure, but the dose of beta-blocker was not. The majority of the review process and the synthesis were well conducted. Despite the absence of a validity assessment, the conclusions appear likely to be reliable.
[Included studies assessed metoprolol, carvedilol, Bisoprolol, bucindolol, nebivolol and atenolol. ]
CRD Commentary: The review question was clear and was supported by specific inclusion criteria. …

Essential hypertension is a heterogeneous multifactorial disease. Data from the National Health and Nutritional Examination Survey and from the World Health Organization have clearly demonstrated that, worldwide, less than 30% of hypertensive patients are adequately controlled by our currently accepted blood pressure goals. Although monotherapy is often unable to achieve blood pressure goals, the use of fixed low-dose combination drugs as alternative treatment seems to be related to a better antihypertensive efficacy and higher response rates in the low range of doses as the result of complementary mechanisms of antihypertensive …
